About Client Choice Services

CCS is helping clients navigate Medicaid, particularly through the TBI (Traumatic Brain Injury) and NHTD (Nursing Home Transition and Diversion) waiver programs in New York. These programs are designed to help individuals with disabilities or seniors receive community-based services rather than institutional care, enabling them to maintain independence and live with dignity.

Eligibility Requirements

Each client must have active Medicaid, need nursing home-level care, and be eligible for either TBI or NHTD based on their unique needs. We met with each client to discuss their program choices, explain the process, and gather necessary documentation from their primary care provider (PCP) and other medical professionals.

The NHTD waiver program is a home and community-based program that helps New York´s Medicaid-eligible seniors and people with physical disabilities receive comprehensive services they need while they live in a community-based setting, rather than in a nursing home, congregate care setting, or other institution

An individual who is: diagnosed with TBI or a related condition, eligible for nursing facility level of care, enrolled in the Medicaid program, and between 18 and 64 years of age, and injured after the age of 18

Information from NYS regarding the CDPAP Program

ALBANY, N.Y. (January 8, 2025) – The New York State Department of Health has released a public service announcement that dispels myths about the April 1 transition of the Consumer Directed Personal Assistance Program (CDPAP) to a single fiscal intermediary. The video features State Health Commissioner Dr. James McDonald speaking directly to New Yorkers who are enrolled in CDPAP.
